Adult Membership

- Regular dental cleanings (2 per year)
- Doctor exams (2 per year)
- Complementary Fluoride treatment
- Routine Radiographs (X-rays) including Panoramic, Full Mouth Series (FMX) & Bite Wings (as needed)
- One emergency visit per year (if needed)
- 20% off bleach trays & whitening
- 15% off Restorative procedures (Fillings, Sealants)
- 10% off Major Procedures (Crowns, Bridges, Cosmetic and Full Mouth Reconstruction)
- 10% off Implant Procedures
- $452 for each additional family member.
- For children under 13, the fee is $390.

Perio Membership

Guidelines
- The membership plan fee is non-refundable.
- Your plan becomes effective immediately upon enrollment.
- Using Financial institutions such as Cherry Financial and/or Care Credit to pay for the plan and finance treatment will incur a convenience fee. For more details, please consult the front office: (770) 428-0434.
- The plan is non-transferable and cannot be assigned to another patient.
- The plan applies exclusively to services provided at La Vita Dental Arts.
The plan is designed for patients without dental insurance. Patients cannot use both dental insurance benefits and other discounts (such as cash or check discount) to pay for the plan. This is not an insurance policy. - Botox, Xeomin and TMJ appliances are excluded from the plan.
- The plan is subject to an annual increase. Benefits must be used before expiration and do not accumulate year to year.
- The plan will auto-renew unless we receive a 60-day notice prior to the plan’s expiration.
